How do I get started? Physicians or their staff may call our office at 301-829-6770 or toll free at 1-877-REHAB14 (734-2214), or fax a prescription to our office at 301-829-6610. Orders can be written for any of the services available to Medicare beneficiaries (see Services page).
Potential patients (or their family/caregivers) can contact their physician for a home health prescription, following the steps above. Our office can assist in this process.
Although receiving the physician's order and patient's demographics (or face sheet) is sufficient, Home Therapy Services has a referral form for convenience. Please call our office and our staff will be glad to fax our referral form, or mail a pad of referral forms, to you.
